Nipple reduction and bilateral gynecomastia reduction, 700 mL


PREOPERATIVE DIAGNOSIS:  Gynecomastia.

POSTOPERATIVE DIAGNOSIS:  Gynecomastia.

OPERATION PERFORMED:
Nipple reduction and bilateral gynecomastia reduction, 700 mL

ANESTHESIA:                                                         General.

PROCEDURE IN DETAIL:                                                The patient was brought to the operating room and placed supine on the operating table.  General anesthesia was administered and the patient was prepped and draped. 

Next 2 incisions were made in both breasts.  Tumescent fluid was then infiltrated.  Tumescent fluid consisted of 1 liter of lactated Ringer mixed with 30 ml of 1% lidocaine and 1 amp of epinephrine.  After this, ultrasonic liposuction was performed at a setting of 5 for approximately 5 minutes on each side.

Next, conventional liposuction was performed with 4, 5 and 6-mm cannulas.  Complete reduction of the breast was performed, as well as the lateral portion, as well as inferiorly.  The same procedure was performed on the contralateral side.  Slightly more was removed from the left side and 700 mL was removed in total. 

After this, the incisions were closed with 5-0 fast-absorbing plain gut.  The nipples were then reduced.  The use of 5-0 Monocryl was used to suture the nipples.  Dressings were applied. 

The patient was then awakened and taken to the recovery room in stable condition.
