POSTOPERATIVE
DIAGNOSIS: Missed abortion.
OPERATION
PERFORMED: Suction D&C.
EBL:
Minimal.
ANESTHESIA: General.
COMPLICATIONS: No.
PROCEDURE: This
is a 24-year-old gravida 6, para 1-0-4-1 with empty gestational sac on
ultrasound. Risks and benefits were
discussed with the patient at length, risks of hemorrhage, infection, and injury
to bowel, bladder, and uterus discussed.
Informed consent was obtained.
The patient was taken to the operating room. General anesthesia was obtained without
difficulty. She was prepped and draped
in lithotomy position in candy-cane stirrups.
A weighted speculum was placed inside the vagina. The anterior lip of the cervix was grasped
with single-tooth tenaculum. The cervix
was already noted to be slightly dilated.
A 9-mm suction curette was advanced to the uterine fundus. The suction device was activated. A moderate amount of products of conception
was evacuated from the uterus. Sharp
curettage followed. A gritty texture was
noted. The suction device was
reintroduced in the site of the uterine cavity.
The remaining clots and debris were evacuated. All instruments removed from the vagina. Hemostasis was noted from the tenaculum
site. The patient was awake and taken to
the recovery room in stable condition.
